What to bring with you for your appointment:
- Current insurance card (including secondary policy if applicable)
- New Patient Paperwork
- Name of your Primary Care Physician
- List of all current medications and/or over-the-counter medication, including dose and frequency
- Pharmacy Name and phone number
- Recent x-rays or MRI, relevant records, if applicable
Insurance and Payment Information
Madison Foot Clinic is committed to providing the highest quality care for our patients while keeping your costs affordable. We accept most major insurance plans as well as HSA, Flex spending accounts, CareCredit®, and prompt pay pricing for clinic treatment and surgical treatment. We are currently not accepting Medicare, Medicare Advantage Plans, or Medicaid.
If you are uncertain about our network status or referral requirements, we recommend you contact your insurance company directly using the number listed on the back of your insurance card to confirm network status to determine specialist benefits.
